Our programs provide coordinated access to a local network of high-quality service providers, including medical & health, financial, legal, housing and lifestyle professionals. Our members have years of experience in their specialty and comprehensive knowledge of available local resources.
WE LISTEN TO YOU
An assessment is a critical first step in helping create the most appropriate life situation for any older adult. Our goal is to understand every aspect of the client’s situation — what is working well and what is not working — from the perspective of the elder, as well as family, caregivers and professionals involved. Members are assigned based on the anticipated needs. Upon completion of the assessment, we will discuss the findings and make recommendations for an action plan.
WE PLAN WITH YOU
An action plan will offer guidance on how best to optimize your life situation and could include referrals to other GYAN members or other trusted community partners. Services include assistance with choosing housing options and with optimizing needed transitions, retirement and estate planning, asset protection, patient advocacy, in-home care planning and identifying available community resources.
WE MANAGE FOR YOU
We offer ongoing relationships to ensure your safety, security and financial needs. Our goal is to maintain or improve your quality of life. Our members will work with you to arrange the needed services at the best possible value.
